What is a symbol digit task?
Each worksheet opens with a key: a row of symbols, each paired with a number. Underneath are rows of the same symbols in a scrambled order, with an empty box under each one. The person works left to right, writing the number that matches each symbol, and the page is scored on how many were completed correctly, how many errors were made, and how long it took. If writing is slow or effortful, the numbers can be said aloud instead while a partner records them.
The format looks like a simple matching exercise, and for the first few items it is. What makes it demanding is the pace. Until the pairs are learned, every symbol means a glance back up to the key, and the eyes have to return to the right spot in the row each time. Speed and accuracy pull against each other for the whole page.

The Symbol Digit Modalities Test, and why this is not it
The format comes from the Symbol Digit Modalities Test (SDMT), a standardized instrument developed by Aaron Smith and published in the 1970s. The test uses a fixed nine-symbol key, a 90-second time limit, and either a written or an oral response, and it is widely used in research and clinical screening of processing speed, notably in multiple sclerosis studies. It is published and licensed by Western Psychological Services, comes with age-based norms, and is administered and interpreted by trained professionals.
Worth knowing: the worksheets on this page borrow the format, not the test. The keys and symbols are different, there are no norms, and nothing here produces a score that can be compared to a population. They are structured cognitive practice materials. They are not assessment, diagnostic, or treatment tools. If someone is going to be assessed with the SDMT, tell the clinician about any symbol digit practice, since familiarity with the format can affect how a score is read.
How the 30 worksheets are graded

A four-symbol key and four rows of eleven symbols. Forty-four items with a key small enough to learn in the first row.
Moderate

A ten-symbol key and five rows of seventeen. Eighty-five items, with similar-looking symbols (two arrows, two triangles) that punish a quick glance.

Using it in a session or at home
- Time it, then count. The convention borrowed from the test is 90 seconds. Stop, count correct items and errors, and write both down. Correct items per minute is the number to watch across weeks.
- Or leave the clock off. Untimed is the right way to learn the format. Add timing once the person is comfortable, not before.
- Say it instead of writing it. If handwriting is slow or effortful, the person says each number and a partner records. The task is the same; the bottleneck is not.
- Watch the rows, not just the total. A first row that is accurate and a last row full of errors says something different from steady errors throughout.
- Move up when it gets easy. When the moderate key is learned by the second row, the ten-symbol pages are ready.
